Three easy ways to schedule your personal "Empowerment
Consultation":
1) Please fill out the
"Empowerment Consultations Information Form"
below; or
2) Send an E-mail to:

3) Call Creative Books and Music, L.L.C. at 1-866-58-CREATE

Please Follow These Steps:
1) When filling out the
"Empowerment Consultations Information Form" below or when sending an E-mail, please include your name, address, contact phone (where you may be reached),
E-mail
address, and the best day and time you may be contacted.
2) You will be contacted via phone or E-mail to set up an appointment time.
At that time, you will need to send in your payment with a signed
"Empowerment Consultation Policy Form."Click here for a copy of the "Empowerment Consultations
Policy Form."
Please fill out, sign, and mail in this
form along with your payment and a photocopy of your Driver's License at least one week prior to your scheduled
appointment.
3) Upon clearance of your payment, you will be contacted for your one-hour "Empowerment Consultation"
at your scheduled time.

for details and important information.
POLICY:

needs to be signed and mailed to Creative Books and Music, L.L.C. at least
one week prior to your
first phone consultation session.
These phone sessions are private and the information is kept
confidential.
Phone appointments are limited. You may also attend Sandra's many presentation events
or register for classes.

PAYMENT
Phone Consultations: $75 (1 hour)
A non-refundable appointment fee of $25.00 is included in the
$75.00 fee. Full payment of $75.00 must be received at least one week prior to
your scheduled session.
You may cancel your session up to three days before the scheduled session
and receive a refund, less the $25.00 non-refundable appointment fee. You would
be refunded $50.00.
At this time, only phone consultations are available based on
schedule availability.
Once your appointment has been scheduled, you will need to send in your payment
or pay with a credit card over the phone. You must also send in a signed
"Empowerment Consultation Policy Form" with a photocopy of
your Driver's License at least one week prior to your first phone consultation session.
Forms of Payment:
You may pay with a credit card over
the phone at least one week prior to your scheduled appointment date and time.
You may also pay
with a check or money order
